He’s the world’s most notorious judge. Simon Cowell has been gracing TV screens for the past three decades, and lots of people have been commenting on his facial evolution.

Back in 2019, Cowell’s friend Piers Morgan made a sardonic Instagram post for his birthday. “Happy 60th Birthday to my (very) old mate @simoncowell. A great guy to whom I owe a lot. But no amount of plastic surgery can change the fact [that] his age now begins with a 6 and mine with a 5.”

Simon Cowell’s looks have definitely turned the heads of almost everyone on the internet as well. “I feel like Simon Cowell from 20 years ago would ridicule and lambast current simon worse than any crazy x factor/american idol contestant,” one Reddit user wrote. “The Now version looks like someone hired a Simon Cowell impersonator from a county fair circuit,” another observed about his current looks.

Did Simon Cowell get plastic surgery?

Simon Cowell has been open about his procedures, but has denied that he’s gotten plastic surgery.  ‘I think it’s hysterical. I mean it just makes me laugh – I haven’t had a facelift or anything,” he said in 2023. “I have had a bit of Botox but kind of nothing drastic. So when I hear about this stuff, honestly it does make me laugh.”

He did get a non-surgical procedure called the Silhouette Soft Lift in 2018.  The procedure is a highly effective thread lift that can be used to correct and tighten the midface, jawline, eyebrow, and neck.

At the time, he said, “For me now, it’s all about having clean skin. If you have clean skin, you look better. But you have to be very careful with some of this stuff.”

In the next couple of years, the American Idol judge revealed that he had an unhealthy obsession with Botox. “There was a stage where I might have gone a bit too far,” Cowell told The Sun in 2022. “I saw a picture of me from ‘before’ the other day and didn’t recognize it as me.”

He admitted he looked “like something out of a horror film.” But, his 8-year-old son helped him steer clear of the injectables. “Eric was in hysterics. Enough was enough. There is no filler in my face at all now. Zero.”

However, in October 2025, a source told Closer UK that the X Factor creator’s friends are concerned about his recent looks. “People around him are whispering he’s spending way too much time at the surgeon’s and needs to put a stop to it now. His pals have been begging him to ease up,” the source dished. “Friends say he brushes off the concern and insists he has to keep up appearances for TV, but the shows he’s doing don’t require that anymore, audiences want authenticity and relatability, not someone who looks like they’re hiding behind a mask.”

 “Simon has told his friends that he now only gets the occasional touch-up but no one believes that, his face changes too drastically,” a source continued. “The irony is he actually looked fine before, he was aging naturally and still had a very handsome presence.”

“Now he looks puffy, stiff and frankly uncomfortable in his own skin, but Simon obviously believes he looks better with all this stuff done to his face. The hope for those close to him is that he takes a real break from the injectables before he does permanent damage to his face.”

The source emphasized that he believes that he needs to keep up with the procedures for his screen time. “But he can’t seem to separate how he looks with his career and even in his circle, where literally everyone has work done, he stands out,” the source concluded. “It’s been heading in this direction for a while but lately it’s crossing the line into waxwork territory.’
